Gozu

Utterly bonkers - its like Takashi Miike (Audition, The Happiness of the Katakuris) does small-townn David Lynch weirdness, and considering Miike films are generally as mad as a box of badgers, thats pretty darn weird. The plot mostly has a small time Yakuza guy searching for the missing corpse of his boss in some strange town populated by freaks. There's a guy with a cows head, some bad transvestites, and lots to do with breast milk and thats before the last fifteen minutes or so when it gets really screwed up. At times during the middle it does become a bit frustrating and slow as the townspeople become increasingly unhelpful in advancing the plot - and many people will just plain hate it because it makes no sense - but for me there were enough stand-out moments and complete unpredictability to make this a work of genius, albeit a very warped genius.
